EVANS, GEORGIA & AUSTIN, TX, MAY 31, 2023 – The Center for Primary Care (CPC), an independent practice that provides comprehensive preventative and specialized care, announced in a press release this week that it has formed a new partnership with agilon health Inc.(NYSE: AGL). According to the statement, the trusted partner empowers physicians to transform health care in the community which will accelerate access to value-based primary care in the area.
CPC will deliver a new model of primary care beginning in 2024 through the partnership to further enhance preventative and chronic care for senior patients.

CPC, an independent primary care practice, operates across eight locations in the Central Savannah River Area, consisting of communities in eastern Georgia and western South Carolina. The center has over 30 primary care physicians that according to the statement, “is committed to preventative health care, as well as the diagnosis and treatment of acute illnesses and chronic conditions.”
With the CPC serving over 76,000 patients, the center said it also offers in-house laboratories and diagnostic imaging services that are made exclusively available to their patients.
“The newly formed partnership with the Center for Primary Care brings agilon’s Total Care Model to even more communities, including senior patients in Eastern Georgia for the first time,” said Steve Sell, Chief Executive Officer, agilon health. “Our team is excited to partner with the Center for Primary Care as they make the move to a full-risk value-based model for their Medicare Advantage patients and primary care physicians.”
The press release stated that “agilon is accelerating at scale the transition to a value-based, Total Care Model for senior patients that improves outcomes and lowers costs.”
The CPC is additionally expected to join agilon’s 2024 class of new partner organizations, including five other physician practices and health systems across America.
Next year, agilon will be in long-term partnerships with 31 physician groups and health systems across 14 states.
